If your Kentucky marijuana card is expiring soon, you’re not alone. Thousands of patients across the state are approaching their renewal date, and many are confused about what the process actually involves. Between the state fee waiver and expanding dispensary access, it’s easy to misunderstand what’s required.
This guide explains everything clearly so you can renew on time and avoid losing access.
Many Cards Are Expiring Right Now
Kentucky’s medical marijuana program officially launched in January 2025, making many of the first certifications that were issued now reaching their expiration date.
Governor Beshear has waived the state renewal fee for 2026 for patients who got their cards in 2025. Because of this, some patients mistakenly believe their card will automatically renew, and others assume they can wait until more dispensaries open before taking action.
Unfortunately, that’s not how the system works.
If you do not return to a licensed practitioner for a new written certification, your card will be deactivated once it expires. That means you cannot legally purchase marijuana from a dispensary until you renew.
The State Fee Does Not Mean Automatic Renewal
The waived $25 state renewal fee only removes the payment requirement for 2026; it does not extend your certification automatically. This is where most of the confusion arises.
To keep an active marijuana card, patients must:
Receive a new written certification
Submit the renewal application to the Kentucky Medical Cannabis Program’s Patient and Caregiver Registration Portal.
Without a new certification and submission to register with the state, your marijuana card and dispensary access expire, even though no state fee is due.
When Should You Renew?
You should plan to renew before your card expires. Waiting until the last minute can create unnecessary stress and potentially cause a gap in access.
We recommend scheduling your appointment at least 30 days before your expiration date. This gives you time to complete your evaluation and ensure everything stays active without interruption. This is especially important since more dispensaries are opening.
If your card fully expires before renewal, you cannot access dispensaries and purchase marijuana until your certification is renewed.
What the Renewal Process Looks Like
To renew, you need to schedule an appointment with a licensed practitioner. During your appointment, the practitioner will review your qualifying condition, discuss how marijuana has been working for you, and answer any questions you may have. Assuming you still qualify, they will issue a new written recommendation.
Once your certification is updated and the state approves your application, your registration remains active. Because the state renewal fee is waived for 2026, you won’t need to submit that payment, but the practitioner visit payment is still required.

What Happens If You Miss Your Renewal?
If you do not renew in time and your card becomes inactive, you will temporarily lose access to dispensaries and can’t legally purchase marijuana until you complete a new certification.
While you can still renew after expiration, it may create a gap in your treatment.
Who Is Eligible to Renew?
If you were previously approved and still have a qualifying medical condition under Kentucky’s medical marijuana program, you are eligible to renew. Conditions such as chronic pain, PTSD, cancer, and other serious medical diagnoses can qualify.
Renewals are required once a year from the date your card was issued. You can find your card’s expiration date directly on your marijuana card to know when it’s time to schedule your appointment. We recommend scheduling your appointment at least 30 days before your expiration date. During your appointment, the practitioner will confirm your eligibility and answer any medical questions you may have.

The state waiving the renewal fee does not mean your card renews automatically. A new written certification from a licensed practitioner and submitting an application through the state portal are still required.
