How to Register for an Account

All passwords should contain:

Minimum of eight characters

One upper-case letter

One lower-case letter

4. Read & accept the Terms & Conditions, select that you are not a robot, then click the “Register” button.​​​​​

3. ​​Select "Individual (If you are applying as a Practitioner/ Patient/ Caregiver)".

5. In your provided email inbox, select the "Verify your NLS Account" email you received and then click “Verify”.​​

​​​​6. Once verified, you will be directed to the Team Kentucky Office of Medical Cannabis’ online portal to sign in.

​​​​​​​​​​​​​​​7. Enter the email and password you set.
​​
​

How to Apply for Your Card
1. To log in, visit https://ky-chfs-public.nls.egov.com/login.
2. Enter the email & password you registered with, read and accept the Terms & Conditions, select that you are not a robot, then click the “Sign In” button.​​​​​

3. Click on “Applications".
4. Click on “CREATE NEW APPLICATION”.

5. Select “New Patient Registration” then click “CREATE APPLICATION”.​​​​​​​​​​​​​​​

6. Complete the “General Information” section by selecting “In-State Qualified Patient”, entering your information in all required fields, and then clicking on “SAVE & NEXT”.​​​​

​​7. Complete the “Applicant Contact Information” section by entering and verifying your “Permanent Residence Address” and “Mailing Address”.​



​​​​​​​​​​​​​8. Complete the “Practitioner Written Certification Information” section by clicking “VIEW AVAILABLE CERTIFICATIONS”, selecting your current certification, and then clicking “UPDATE”.​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​

9. Complete the “Caregiver General Information” section by answering if you plan to use a caregiver. Click “SAVE & NEXT”.​​​​​​​​​​​​​​​​​​​​​​​​​​​​

​10. Complete the “Documents” section by clicking “UPLOAD”.​​​​​​​​​​​​​​​​​​

​​11. Click “DROP A FILE HERE OR CLICK TO SELECT” to add your Notarized Signature Page.​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​
12. Click “UPLOAD”.​​​​​​​​​​​​​​​​​​​​​​​​​​​​


13. Click “SAVE & NEXT”.​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​

14. Complete the “Question and Attestations” section by answering all attestations, typing your full legal name under "Signature", and then clicking on “SAVE & NEXT”.

16. Review your answers. Click “PAY & SUBMIT”.​​

After payment has successfully been made, you MUST wait for an email a no-reply email from kychfs-noreply@ky-chfs.egov.com letting you know your application has been approved.​​
